We are looking to recruit for a Advanced Nurse Practitioner, (ANP) to join our team in Cowplain, Waterlooville to lead the delivery of Enhanced Access appointment provision for the local Primary Care Networks (PCNs).
Hours: Friday 18.30 – 21.30
Advanced Nurse Practitioner Duties and Responsibilities:
- Assess undifferentiated patients with a range of acute, non-acute and chronic medical conditions
- Take history, make appropriate physical examinations, formulate differential diagnoses and formulate/manage treatment plans when working in any clinical environment
- Refer and facilitate patient pathways through appropriate referrals to primary, community, secondary and tertiary care for patients when necessary, such as 2-week wait, urgent and routine referrals to specialities
- Prescribe medication as required within your clinical competence
Recognise the need and initiate emergency treatment to save life or to safeguard the interests of the patient and refer to emergency services when appropriate
Advanced Practitioner Skills & Experience:
- 3 years of experience working in a primary care GP practice
- 3 years’ experience in an urgent care setting such as an urgent treatment centre
- At least 6 years post qualification
- Excellent knowledge and experience in the assessment and management of minor injury and illness presentations
- Good knowledge of primary care referral pathways such as 2-week-wait referrals
- Registered with the NMC/HCPC
- Non-medical prescriber
- Post graduate diploma (PgDip) and working towards completion of the MSc qualification. Completed level 7 modules must include a history taking and patient examination/decision-making module
